Purpose: The purpose of this study is to evaluate the long-term results of transtrochanteric rotational osteotomy of femoral head (RO, Sugioka's osteotomy) for severe Legg-Calve-Perthes' Disease (LCP). Method: Twenty-nine hips in 27 patients were retrospectively reviewed. The mean age at operation was 10.2 years old. The mean follow-up period was 11.2 years. Six hips were Catterall Class III and 23 were Class IV. The initial conservative treatments were ineffective. All the patients did not have indications for varus osteotomy because of hinge abduction, etc. Results: Average Harris Hip Score was 95 points at the final follow up. Fifteen hips (56%) had good radiographical outcome with the modified Stulberg's classification. Better outcomes were obtained in patients with younger age, more bone formation or preservation of lateral and posterior ridge of epiphyses and less subluxation of the femoral head. Conclusions: RO is an effective procedure for severe LCP. Age, epiphyses reconstitution and subluxation of femoral head had influences on surgical outcomes.
